Acknowledgements

My child will be participating in the 2024 Oshkosh West Bands trip to New York City during spring break 2024.

I accept.

I understand the financial obligations of the trip including the payment schedule. Further, I acknowledge that Bob Rogers Travel requires trip insurance to be purchased in addition to the standard trip payments in order to be eligible for refunds should the trip be canceled or postponed. I understand that my student will not travel with outstanding band fees/fines of any sort. *

I accept.

I understand the availability of refunds. I also understand that fundraising profits cannot be returned for any reason. *

I understand that notification must be submitted in writing (not by phone or email) on the cancellation form to be removed from the trip and to be eligible for refunds if enrolled in CFAR trip insurance. *

I understand that trip insurance has been offered by the travel company at additional cost to cover unforeseen circumstances in the case of a student not being able to travel on the prescribed dates. *

I understand that this is a performing trip and my child will attend all Sunday rehearsals in February and March of 2024. I further understand that failure to attend rehearsals may prevent my student from traveling and that refunds will be available based on the standard policy. *

I understand that my student must pass first semester band class with at least a 75% average and be passing second semester with at least a 75% average at the time of travel. I understand that refunds will be available based on the refund schedule. *

I understand that my child and at least one guardian must attend the Pre-Trip Meeting prior to departure. *

I understand that a major violation of OWHS/OASD rules prior to the trip may result in the student being ineligible to travel with the band on the trip. Refunds will be available based on the standard policy. *

I understand that a violation of OWHS/OASD rules while on the trip will result in disciplinary action. *
